SCFHP joins HMIS
As of June 2020, SCFHP has partnered with the Office of Supportive Housing of Santa Clara to participate in the Coordinated Assessment System and administer the Vulnerability Index-Service Prioritization Decision Assessment Tool (VI-SPDAT). The information provided in the assessment will be entered into the Homeless Management Information System (HMIS), a secured database that will determine the best housing intervention for our members who are at risk of or experiencing homelessness. It will also allow us to partner with the network of other community-based organization to close gaps in care. SCFHP is pleased to be a part of this partnership and looks forward to working with other community providers to address homelessness.
