Have you completed your ICT training?

If your SCFHP patient is eligible for Case Management, you will receive an invitation to attend Interdisciplinary Care Team (ICT) meetings. Completion of our ICT course ICT Training - Core Competencies is required as part of the on-boarding and contracting process for all SCFHP Cal MediConnect providers and participating Medi-Cal providers, and it must be completed before attending an ICT meeting, or annually.
Why is the ICT important?

  • ICT focuses on the coordination of psycho-social, medical, pharmacy, and behavioral health services to assist patients in navigating the health care system and ultimately improve their health outcomes.
  • It helps coordinate care between the patient or authorized representative, family or caregiver, case manager, primary care physician, and other participants, including specialists, pharmacists, discharge planners, providers of long-term services and supports (LTSS), and behavioral health.
  • It facilitates care management, which includes assessment, care planning, authorization of services, and transitional care issues.

Getting started
Every SCFHP Cal MediConnect patient has an assigned Care Coordinator, who will work with you to monitor patient participation in SCFHP’s Case Management programs. You will also receive copies of the patient’s Individualized Care Plan (ICP).
