Excellence in facility site review

Congratulations to Dr. Hahn Nguyen, Dr. Nayyara Dawood, and Dr. Nicole Thai, who recently had exempted passes on DHCS’ required facility site review (FSR) and medical record review (MRR)! They each scored above 90% on the FSR without any critical elements, including no infection control or pharmacy deficiencies, and scored above 90% on all sections of the MRR. In addition, Dr. Sima Stein, Dr. Dat Huynh, and Dr. Nhi Huynh had exempted passes on their MRR, and Dr. Juan Posada had an exempted pass on his FSR.
