The SCFHP multidisciplinary Case Management team consists of RNs, LCSWs, and other trained health care specialists that work in collaboration with PCPs and specialists. The case manager or case coordinator helps your patient manage their medical, mental health, and/or psychosocial needs, and coordinate their services.
The team reviews all new case management referrals to determine the appropriate level of intervention. Depending on whether the patient is an SCFHP Cal MediConnect or Medi-Cal member, a mandated health risk assessment (HRA) is completed. An Individual Care Plan (ICP), including prioritized goals and interventions, is developed for the patient upon successful completion of the HRA.
SCFHP values your involvement in developing care plans. The Case Management team will fax completed ICPs to the assigned primary care provider (PCP) for input on the care plan. Changes to the ICP can be shared with the patient’s SCFHP case manager by faxing to 1-408-874-1432. PCPs are also invited to Interdisciplinary Care Team (ICT) meetings for their patients for more complex care management needs.
