Tools and guidelines for FSR and MRR
The Department of Healthcare Services (DHCS) has released a new All Plan Letter (APL) 22-017 which supersedes APL 20-006 and PL 14-004. The APL states that all PCP sites participating in the Medi-Cal Managed Care Program and the Medicare-Medicaid Plans are required to complete an initial review and subsequent periodic full scope reviews consisting of three (3) components:
- Facility Site Review (FSR)
- Medical Record Review (MRR)
- Physical Accessibility Review Survey (PARS)

DHCS has updated the Facility Site Review and Medical Record Review standards and criteria effective July 1, 2022. The tools and guidelines are used to assess provider offices on how well they meet regulatory requirements. These DHCS updates align with current guidelines and restructured criteria groups to better help identify deficiencies. Based on the FSR and MRR findings, PCP sites must complete Corrective Action Plans (CAPS) within DHCS CAP timelines for cited deficiencies.

PARS is a physical evaluation of provider sites to assess the level of physical accessibility and provide this information to Seniors and Persons with Disabilities (SPDs).

SCFHP monitors when providers are approaching their next FSR and connects with providers individually to schedule a walkthrough. The walkthrough covers details of the FSR and how it may have changed from its prior iteration.
“Disability Rights and Provider Obligations” is not included in the FSR video linked above but is required employee training by DHCS.
