This search tool will help you find a provider or doctor who accepts Medi-Cal, Healthy Kids, or Cal MediConnect. You can also search for facilities, labs, and hospitals.

A prior authorization or referral may be required to access some providers. Even if a provider is listed here, it does not mean you are guaranteed services for benefits from that provider. SCFHP’s provider network is subject to ongoing changes.

Some hospitals and other providers do not provide one or more of the following services that may be covered under your plan contract and that you or your family member might need: family planning; contraceptive services, including emergency contraception; sterilization, including tubal ligation at the time of labor and delivery; infertility treatments; or abortion. You should obtain more information before you enroll. Call your prospective doctor, medical group, independent practice association, or clinic, or call the health plan at 1-800-260-2055 to ensure that you can obtain the health care services that you need.

We have free interpreter services to answer any questions you may have about our health or drug plan. To get an interpreter, call Customer Service. Someone who speaks your language can help you. This is a free service.

Santa Clara Family Health Plan Cal MediConnect Plan (Medicare-Medicaid Plan) is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees.
