Disability access

Santa Clara Family Health Plan (SCFHP) is committed to serving all our members, regardless of disability or other physical limitation.

Physical access

The SCFHP office location is accessible to people with disabilities. So are many healthcare provider offices. Accessibility information for each provider office can be found in our Find-a-Doctor tool. If you need help finding a provider’s office that you can access, please call Customer Service at 1-800-260-2055.

Access for those with hearing or speech impairment

SCFHP uses the text telephone device (TTY, also known as TDD) number through California Relay Services to help callers who need it. To use the TTY, call 711.

Access for those with blindness or low vision

You can get important plan materials in large print, braille, and digital formats. For materials in any of these formats or for help with reading our materials, please call Customer Service at 1-800-260-2055.
