What is Medi-Cal?
Medi-Cal is California’s Medicaid program for people with limited income and resources. It provides health insurance options to low-income adults, kids, families, seniors, pregnant people, and people with disabilities. Medi-Cal eligibility is based mainly on your income, and in some cases, your property.
Use the Shop and Compare tool on the Covered California Medi-Cal website to see if you or your family may be eligible for Medi-Cal.

Easy to apply for benefits
You have options when it comes to applying for Medi-Cal:
- Through application assistance at the SCFHP Blanca Alvarado Community Resource Center
- Through Covered California
- Through MyBenefits CalWIN
- Through an eligibility and enrollment specialist at Santa Clara County Social Services

Watch your mail for information
Once your application is processed, simply wait for a letter informing you whether or not you qualify for Medi-Cal. If you do qualify, you will receive a Medi-Cal benefits identification card. You will also receive a Medi-Cal Choice Form to choose your health plan.

Choose Santa Clara Family Health Plan
When you are enrolling, select us as your health plan on your Medi-Cal Choice Form. Once we confirm your enrollment, we will mail you a welcome packet.
If you do not choose a health plan, one will be chosen for you.
Do you qualify for both Medicare and Medicaid?
NEW: SCFHP DualConnect (HMO D-SNP) Plan
We now offer a Dual Eligible Special Needs Plan (D-SNP) for individuals who are entitled to both Medicare and Medicaid. Read more about SCFHP DualConnect and see if you qualify.
Keep yourself and your family covered
It is very important to share your correct mailing address, phone number, and email address contacts with the county for updates about your Medi-Cal health plan coverage. Your updated contact information is how the county sends you your Medi-Cal Redetermination or Renewal Packet every year.
If you receive a Medi-Cal Redetermination or Renewal packet in a yellow envelope, make sure you complete it and send it back to keep your coverage. For more information about redetermination, visit www.scfhp.com/renew.
