About us
Santa Clara Family Health Plan (SCFHP) is a local, community-based health plan dedicated to creating opportunities for better health and wellness for all. In partnership with providers and community organizations since 1997, we work to ensure everyone in Santa Clara County has access to equitable, high-quality health care. With a strong commitment to integrity, outstanding service, and support for our community, we serve more than 320,000 people through our Medi-Cal and SCFHP DualConnect (HMO D-SNP) health care plans.
Our mission
SCFHP's mission is to improve the well-being of our members by addressing their health and social needs in a culturally competent manner, and partnering with providers and organizations in our shared commitment to the health of our community.
Our vision
Health for all – a fair and just community where everyone has access to opportunities to be healthy.
Our values
Our strategic plan
With alignment to our mission, vision and values, SCFHP’s 2021-2023 strategic plan advances our efforts to ensure that all in our community have access to high quality health care and to eliminate the unacceptable health disparities that still impact too many. Our strategic plan goals are:
COMMUNITY HEALTH LEADERSHIP
Be a recognized local leader and collaborator in improving the health of communities impacted by disparities
QUALITY, ACCESS AND EQUITY
Deliver exceptional quality outcomes and health equity for all Plan members.
ORGANIZATIONAL EXCELLENCE
Consistently demonstrate administrative and service excellence.
