Your health plan has two offices that you can visit. The main office is at 210 East Hacienda Avenue in Campbell. If you have questions about your health insurance our member services staff will help you. You can also call Member Services at 1 (800) 260-2055. The hours at our Campbell office are Monday - Friday, 8:30 am to 6:00 pm.
If you live on the East side, you can also go to the Family Resource Center at 1775 Story Road, Suite 130, (Mi Pueblo Plaza shopping center), San Jose, CA 95122. The staff at the Center invites you to visit. The office has been newly expanded so there is more room for helping our members.

New Member Orientations to help you learn more about your benefits.
Application Renewals.
Free Health Education Classes and literature. Topics include nutrition, diabetes, parenting and how to stop smoking.
Free Bicycle Safety Classes (free helmets to children - ages 6 to 14 - who finish the class).
The hours at the Family Resource Center: Monday, Tuesday, Thursday and Friday: 8:30 am to 1:00 pm and 2:00 to 6:00 pm Wednesday: 10:00 am to 1:00 pm and 2:00 to 6:00 pm
