Watch for a renewal letter in the mail.You will receive a letter in the mail asking you to fill out a renewal form.
This letter may come from Healthy Families, Medi-Cal, or the Social Services Agency, or the Santa Clara Family Health Plan (for the Healthy Kids program and for Healthy Generations).

address or telephone
number changes. This way
the renewal letter will get to
you without delay, and you
will be able to keep your
benefits without an
interruption.
Also, you are always welcome to visit one of our offices. You can call to make an appointment, or you can just drop into our offices.
