Health Education

Dedicated to helping our members stay healthy and happy, we provide facts and services that enable people to understand and manage their health. We partner with a number of agencies within the community to provide health education classes and programs that best meet the needs of our membership. Health Education services are designed to help support our members live healthier. Services include:

24-hour Interpreter
Health Education Programs

All classes and programs are provided free of charge to Santa Clara Family Health Plan members. (Before attending a class, be sure to fill out a Health Education Referral Form). If you require help in a language other than English, 24-hour interpreter services are available.
