Medi-Cal
Free for families who qualify. You may be eligible if:
• You have children or are pregnant or aged or blind or
disabled
• You’re a U.S. citizen, national, or legal immigrant
• You’re a Santa Clara County resident
• You have a household income within Medi-Cal guidelines.
For example:
Family Size: 2 (parent and child); Income: up to $1,070 to
$2,139 per month
Family Size: 5; Income: $1,885 to $3,769 per month

Healthy Families
Low-cost for children, ages one through 18.
$4 - $15 a month per child; $45 per family per month maximum.
You may be eligible if:
• You’re a U.S. citizen, national, or legal immigrant
• You’re a Santa Clara County resident
• You have a household income within Healthy Families guidelines.
For example:
Family Size: 2 (parent and child); Income: up to $2,673 per month
Family Size: 5; Income: up to $4,711 per month

Healthy Kids
Low-cost for children, ages one through 18, regardless of immigration status. $4 - $6 a month per child; $18 per family per month maximum.
You may be eligible if:
• You’re a Santa Clara County resident
• You have a household income within Healthy Kids guidelines.
For example:
Family Size: 2 (parent and child); Income: up to $3,207.50 per month
Family Size: 5; Income: up to $5,652.50 per month

Healthy Generations
Healthy Generations was designed for Santa Clara County residents who receive Medi-Cal and have Medicare Part A (hospital insurance) and Part B (medical insurance). Healthy Generations is a Medicare Advantage plan, and includes Medicare Part D prescription drug coverage.
