Do You Qualify?

Some basic qualifications:

  • Uninsured children from birth to age 19
  • Do not qualify for No-Cost Medi-Cal OR the Healthy Families Program
  • Without job-based coverage for 3 months prior to applying for program (unless certain exceptions apply)
  • Live in Santa Clara County
  • Regardless of immigration status
  • Income guidelines are based on family size

For a family of four:

Child's Age Monthly Income
Birth up to 19 $0 to $5,163

You may qualify even if your income is over the guidelines above. Family income will be adjusted by taking deductions if you pay for child support and/or alimony, or pay for childcare.

Under 18? Some minors may apply for Healthy Kids for themselves and their children.
