WHAT IS HEALTHY GENERATIONS?

Medicare and Medi-Cal Made Easy for You
Healthy Generations combines your Medicare and Medi-Cal benefits into one single plan, including Medicare Part D prescription drug benefits. This makes it easy for you to get the healthcare you need, working with just one health plan and one set of providers. We make sure that the care you get is thoughtful, with open communications and a shared commitment to your health.

Healthy Generations has formed a network of doctors, specialists, hospitals and pharmacies. You can only use doctors who are part of our network. The health providers in our network can change at any time.

Healthy Generations was designed for people who are eligible for Medi-Cal and Medicare. That means you must be eligible to receive Medi-Cal and have Medicare Part A (hospital insurance) and Part B (medical insurance). You must live in Santa Clara County and use Healthy Generations’ contracted providers for all of your routine care.

Healthy Generations is a Medicare approved HMO. Santa Clara Family Health Plan is authorized to refuse to renew its annual contract with Centers for Medicare & Medicaid (CMS), the federal agency that runs the Medicare program. CMS may also refuse to renew the contract. That termination or non-renewal may result in termination of the beneficiary's enrollment in Healthy Generations. In addition, Healthy Generations may reduce its service area and no longer offer services in the area where the beneficiary resides. If any of these occur, we will send you advance notice of the changes and your options.

Benefits, formulary, pharmacy, network premium and or co-payments/co-insurance may change from one calendar year to another. Please contact Healthy Generations for details.
