Comprehensive Formulary - 2008
The Healthy Generations Formulary is a list of drugs selected by Santa Clara Family Health Plan in consultation with a team of health care providers. The formulary includes the prescription drugs believed to be a necessary part of a quality treatment program. We will generally cover any drug listed in your formulary as long as the drug is medically necessary, the prescription is filled at a Healthy Generations plan pharmacy, and other plan rules are followed.
