How Much Does It Cost?

Monthly Premiums

  • $4 to $15 per child for Santa Clara Family Health Plan
  • A family will never pay more than $45 per month no matter now many children are enrolled
  • If you pay for three months in advance, you get the fourth month for free

Co-Payments (Co-Pays)

  • NO co payments for check-ups and other preventative services
  • $5 co-pays for some services
  • No family will pay over $250 per year in co-pays
