Case Manager - Special Populations
Oversee development and implementation of assessment and outcome measures for the case management program for special risk population. Perform comprehensive case management of SCFHP’s Agnews Transition program. Serve as the liaison for SCFHP members served by SARC programs. Assist in the development of disease management strategies to support persons with disabilities and chronic conditions to improve healthcare outcomes. Requirements: Current California RN (Registered Nurse) license, Minimum 3 years’ hands-on experience in acute car, Minimum 1 year in Managed Care, Utilization Management or Case Management, Valid California Drivers License, and basic working knowledge in computer software, such as Word, Excel and PowerPoint. FRC Client Service Specialist
Support a wide variety of complex services required to navigate programs that provide health coverage for visitors of the SCFHP Family Resource Center. Develop and maintain trusting relationships with families from diverse ethnic backgrounds to enhance and retain enrollment in the Plan's various programs, including all lines of business of the Plan plus other health access programs. EOE
Requirements: BS/BA degree. Major in Social Services, Education, Public Administration and Public Affairs or closely related field or equivalent experience. Minimum 4 years' experience in health services, government relations, case management, customer service or related. Adept in Project Management. Must be Bi-lingual in Spanish/English or Vietnamese/English.
